Goodwill gift card request form from the Children’s Guardian Fund

For children placed in Relative or non-relative care, or reunified with their parents, the assigned Guardian ad Litem can request a $100 Goodwill gift card to be utilized at any Goodwill location in Manatee, Sarasota, or Desoto counties. The gift card can be used to purchase items such as clothing for the child, furniture, bedding, housewares, and other necessities needed to make the child feel comfortable in their new residence.

Please complete this form to submit your request to the Children’s Guardian Fund office. The Goodwill $100 gift card will be sent to the Guardian ad Litem’s address, you can deliver it in person during your next home visit.
