If the request is for more than one child, please list all children on one form. Do not fill out separate forms if the children are part of the same case. The request will be submitted to our community partner for a refurbished unit, and when it is ready, the unit will be delivered to your CAM. Our partners are slowly opening back up, but they have a backlog. This will cause delays. Thank you for your patience.

The request will be submitted to our community partner for a refurbished unit, and when it is ready, the unit will be delivered to your CAM. This process normally takes 1-2 weeks. All requests will be forwarded to your CAM for approval.
