Guardian ad Litem request for Birthday Gift Card from the Children's Guardian Fund

Children’s Guardian Fund is posed to provide a Walmart birthday gift card to celebrate a child’s birthday. All children are eligible for a birthday gift card with a value of $35.

You may submit your birthday gift card request up to 2 months in advance. For example, in September you may request October and November birthday gift cards.

We process birthday gift cards on the 1st and the 15th of each month. The card will be mailed 2 weeks prior to the child's birthday to the address provided below.

The birthday gift card will be mailed to the address provided two weeks before the child's birthday. This request will be forwarded to your CAM for approval.
